 The role of Active Cornwall

We champion the benefits of incorporating movement and physical activity into everyday life, focusing on those facing the most significant inequalities. 

Sharing our unique insights, knowledge and resources, we work collaboratively with others to influence policy and help integrate 
activity into education, healthcare and community life.

We are one of 43 independent Active Partnerships in England, collaborating with national partners and ‘Uniting the Movement’ 
for Sport England in Cornwall, tackling inactivity and reducing inequalities.
 

In partnership with the NHS, Cornwall Council and local delivery partners, we strategically commission and build quality-assured capacity in priority places for those who need our support the most. By connecting with communities, we tackle barriers that prevent people from leading more active lives, powering positive and sustainable change at scale, and influencing active environments.

About The Role

Role Purpose as Health Lead

You will lead a number of key work areas to support this work, including:

• Realise the potential of physical activity and movement as a tool to transform patient outcomes in the prevention, treatment and recovery of long-term health conditions, developing opportunities for physical activity to contribute to local ICS priorities/plans and the NHS Forward Plan
• Integrate physical activity and movement into health and care systems/pathways locally with a focus on support for delivery of Anticipatory Care, Personalised Care (health coaching, social prescribing and supported self-managed care) and Population Health Management
• Empower health and care professionals (HCP), building capacity and advocacy for physical activity and movement by addressing the key barriers in enabling the workforce to engage patients/clients in brief interventions regarding physical activity behaviour change
• Support the ongoing integration and transformation of health, care and VCSE (voluntary, community and social enterprise) sectors, using physical activity and movement as the conduit for connecting more of the system to itself with a focus on targeted place-based models of care
